Médecins Sans Frontières (MSF), also known as Doctors Without Borders, is one of the world’s leading independent medical humanitarian organisations. Working in over 70 countries, MSF provides emergency medical care to people affected by conflict, epidemics, natural disasters, and barriers to healthcare access.
With thousands of medical and operational staff deployed globally, continuous training is critical to ensure safe, high-quality care in complex and often volatile environments. Staff must be able to access learning quickly and reliably — frequently in low-resource or low-connectivity settings.
As MSF’s operations expanded, their training needs became more complex and geographically diverse. However, their existing Moodle-based platform was no longer equipped to support the scale, flexibility, and localisation required to sustain their global mission.
The Challenge: Modernising a Legacy LMS to Meet the Demands of a Worldwide Workforce
Médecins Sans Frontières (MSF) needed to strengthen its digital learning infrastructure to support a rapidly growing global workforce operating in high-pressure, resource-constrained environments. Four core challenges emerged:
1. User Experience for Frontline Teams
The existing Moodle platform was not sufficiently intuitive for busy, non-technical healthcare professionals working in the field. Accessing critical training was not always seamless, creating friction at moments where speed and clarity were essential.
2. Administrative Burden and Limited Automation
Manual enrolments, communications and reporting processes placed unnecessary strain on administrative teams. The lack of advanced automation reduced operational efficiency and limited MSF’s ability to respond quickly to evolving humanitarian needs.
3. Multi-Region and Multilingual Complexity
Operating across diverse geographies, MSF required a platform capable of supporting multiple regions, governance structures and fully localised multilingual training while maintaining global consistency and quality standards.
4. Scalability for Global Growth
The legacy architecture was not designed to support long-term expansion. To continue delivering life-saving training worldwide, MSF required a robust, flexible and future-ready LMS that could scale alongside its growing humanitarian operations.
To continue providing life-saving training in the field, MSF needed a robust, flexible, and globally scalable Moodle Learning Management System (LMS).

The Solution: A Custom Moodle LMS for Humanitarian Impact
Titus partnered with MSF to deliver a fully customised Moodle LMS solution, designed to support their global operations, streamline training delivery, and reduce complexity for administrators and end users alike.

1. Seamless Content Migration
A major part of the project involved the secure migration of content from MSF’s legacy Synergy platform to their new Moodle LMS. Our team provided:
- Full data extraction and backups
- Clean migration of courses, resources, and records
- Rigorous testing to ensure accuracy and usability
This ensured minimal disruption and fast time-to-value.
2. UX-Focused Platform Design
To drive adoption and engagement across a diverse global user base, Titus implemented a bespoke Moodle theme, built with UX best practices. The goal: create a platform that feels intuitive and requires little to no training.
The result? Faster onboarding, reduced learner frustration, and improved course completion rates.
3. Advanced Functionality for Real-World Needs
We enabled a range of smart features to meet MSF’s operational goals, including:
- Integrated Zoom plugin for live, in-platform video training, ideal for time-critical learning sessions
- Certification workflows to automate enrolment and credentialing, crucial for maintaining medical compliance
- Multi-tenancy support, giving different departments and countries autonomy over their own learning environments
4. Ongoing Consultancy and Support
Titus delivered hands-on training and consultancy to MSF’s internal teams. We helped them unlock the full potential of Moodle with guidance on:
- Custom features and integrations
- Scalable user management
- Reporting and learning analytics
Results: Global Medical Training, Streamlined and Scalable
With their new custom Moodle LMS, MSF can now:
Deliver consistent training across 70+ countries
Streamline automation
Reduce onboarding time
Monitor progress, certification and compliance with confidence
End-User Impact: A Seamless Learning Experience for MSF Teams
The success of the platform is best reflected in the experience of learners on the Global Health & Humanitarian Medicine (GHHM) programme. Designed for MSF’s global workforce, the LMS provides an intuitive and accessible way for participants to engage with course content, complete modules, and track their progress.
With improved usability, multilingual capability and mobile-friendly access, learners can focus on developing their knowledge and skills, ensuring the programme continues to strengthen expertise across MSF’s international teams.
